Transcript Release Form - paper copy printable

 
1. Current students can have up to three Official Transcripts processed for no cost. Any more than three, the fee is $4 per transcript. Complete the form and return to Mrs. Kuykendall in the Counseling Office.
 

2. Past Graduates will need to pay $4 per transcript. Complete the form and mail to the address on the form along with the $4, or if you are local you can stop by during school hours with the form and pay and either turn it in or wait to take with you.
